Since 2020, Oslo Economics, in collaboration with the University of Oslo and several experts, has conducted an evaluation of interventions in general practice on behalf of the Norwegian Directorate of Health. The evaluation shows a positive development in recent years. The measures introduced have been important for improving the situation, but they have also incurred significant costs.
Users are generally satisfied with the quality of the service. The number of people without a GP has declined and, as of spring 2025, is at its lowest level since 2021. The years 2023–2025 saw the highest net increase in GPs since the scheme was introduced. Municipalities report improved recruitment, and more medical students and LIS1 doctors now express an interest in becoming GPs. GPs report lower workloads and greater financial security. GP offices are becoming fewer and larger, and the use of digital tools is increasingly common.
At the same time, important challenges remain. Fewer than half of residents find the waiting time for appointments acceptable. There is limited information about the quality of services, and municipalities have few tools for obtaining operational data. Geographic variations are substantial, and the situation remains most difficult in rural areas.
The measures implemented appear to have been crucial for improving the situation, but they are also costly. The combined annual costs of increased capitation payments, the National ALIS scheme, supervision, and ALIS offices are estimated at NOK 1.8 billion in 2024. Additional costs arise from other national and municipal initiatives. Going forward, rising demand for GP services will require more efficient use of resources.
